APPLYING FOR A CERTIFICATE OF CRIMINAL RECORD

Eligible Applicants


  • Registered residents of Miyagi
  • Most recent registered residence in Japan is in Miyagi

With regard to the preparation of the application


  • We need to collect your fingerprints for the application. Applicants must come to the office in person.
  • The Certificate of Criminal Record is written in 5 languages (Japanese, English, French, German and Spanish).
  • Depending on the requesting authority and the reason for the request, there will be some cases where may not accept your application immediately. Please call and check with us in advance.

Office hours


9:00 a.m. to 11:30 a.m. / 1:00 p.m. to 4:30 p.m. on weekdays (Monday to Friday)
Closed on Saturday, Sunday, national holidays, holidays at the end and beginning of a year

3-8-1. Honcho Aoba-ku Sendai, Miyagi (east side of the Miyagi Prefectural Government building)

Nearest subway station: "Kotodai-koen Station" (Exit North #2) on Namboku Subway Line
*You cannot apply at a police station.


Documents required for application


For registered residents of Miyagi

  1. 1.Any one of the following documents that shows your name and address:
     -An official copy of your Certificate of Residence (issued within 6 months)
     -Driver's License
     -Residence Card
     -Special Permanent Resident Certificate
     -Individual Number Card (My Number Card) ※Notification Card for Individual Number is not accepted.
  2. 2.Passport ※a copy is not accepted
  3. 3.A document that confirms the necessity of the certificate issued by a government agency.
    (e.g. request letter from embassy, completed visa application form, etc. )
    If you have a digital file please print the document beforehand and bring it.
    Documents on your smartphone or laptop are not accepted.

For overseas residents

  1. 1.Certificate of Residence deregistration or address history attached to the family register (issued within 6 months)
  2. 2. Documents which verify your current overseas address (overseas driver's license, mail addressed to the applicant)
  3. 3. Passport ※ a copy is not accepted
  4. 4. A document that confirms your necessity of the certificate issued by a government agency.
    (e.g. request letter from embassy, completed visa application form, etc.)
    If you have a digital file please print the document beforehand and bring it.
    Documents on your smartphone or laptop are not accepted.

Service Fee


Miyagi Prefecture revenue stamps (Shunyushoshi) for 450 yen

You cannot pay in cash for this certificate.

Revenue stamps are not available at Miyagi Prefectural Police H.Q. Please purchase them in advance.

Picking up your certificate


Your Certificate of Criminal Record will be available approximately 7 to 10 days from the day of application.
The certificate will be given directly to the applicant in person. The certificate cannot be mailed.
If you wish to have someone else to pick up your certificate, please let us know when you apply.
In the event that an authorized agent is to pick up the certificate, that person must show both of the following:
 -Power of attorney authorization letter
 -Any photo identification issued by Japanese authorities of the agent(e.g. driver's license, passport, residence card)
