| 犯罪経歴証明書（海外渡航証明）の申請について APPLICATION FOR A POLICE CLEARANCE CERTIFICATE |

| 宮城県警で申請ができる方は、次のとおりです。 Police clearance certificates by the Miyagi Prefectural Police Headquarters are issued only to as follows: |

| （１） | 宮城県に住民登録（外国人登録）をしている方 Residents of Miyagi (or Alien Registration Certificate(A.R.C.) holders registered as residents of Miyagi. |

| （２） | 住民票が除票（外国人登録が閉鎖）になっている方で、日本における最終の住民登録（外国人登録）が宮城県にあった方 Ex-residents (or, if you are non-Japanese, ex-holders of an A.R.C.)with Miyagi as the very last registered place of residence. |

| ☆ | 申請の際には指紋を採取します。必ず申請者が来てください。 Applicants must apply in person as the procedure requires fingerprinting. |

| ☆ | 現在、日本国外に居住している場合は、居住している国（地域）の日本大使館・領事館で申請することができます。 If applicants live abroad, they can obtain the certificates from their local Japanese embassy or consulate of their countries (or regions). |

| ※ 婚姻等で氏名・本籍を変更している方のうち、パスポートの記載事項を変更していない方は、記載事項変更後に申請されるか、もしくはその事実が確認できる書類をお持ちください。 If you have changed your name or permanent residence due to marriage etc. and have not updated there changes on your passport, you should apply after updating your passport or bring any document that verifies your current status. |

・ 申請日から、約１週間から１０日後のお渡しとなります。 Your police clearance certificate will be made available between about 7 to 10 days after the day of application. ・ 申請者に直接お渡しします。 The police clearance certificate will be issued directly to its applicant. ※ 郵送はできません。 Certifications can not be mailed to applicants. ※ 代理受領希望の方は、申請時にお申し出ください。 If you wish someone else to pick up your certificate, please let us know when you apply. |
